New Enhancements of the Transverse Scrotal Surgical Technique for Placement of the Artificial Urinary Sphincter Allows More Proximal Placement of Cuffs

چکیده

Introduction: Traditional artificial urinary sphincter (AUS) implantation requires the patient be placed in lithotomy, a perineal incision for cuff placement, and a inguinal incision for reservoir and pump placement. We believed AUS could also be placed more through one transverse scrotal incision (J Urol I 69:26 1, 2003). Cuffs placed in this procedure were usually 4.0 cm. Questions about the efficacy of one transverse scrotal incision have focused on the frequency of 4.0 cm cuff placement. To effect a more proximal cuff placement in transverse scrotal approach, we report enhancements to the original technique with improved proximal cuff placement and resultant use of a preponderance of 4.5 cm cuffs.

Re: New surgical technique for sphincter urinary control system using upper transverse scrotal incision.

PURPOSE Traditional implantation of the AMS Sphincter 800 Urinary Control System (American Medical Systems, Minnetonka, Minnesota) requires 2 incisions. The cuff is placed via a perineal incision, and the pressure regulating balloon and pump are placed through a separate suprapubic incision. We describe a novel implantation of all the artificial urinary sphincter components using a single upper...

Perineal approach for artificial urinary sphincter implantation appears to control male stress incontinence better than the transscrotal approach.

PURPOSE Traditionally cuff placement of an artificial urinary sphincter is done through a perineal approach. A new approach through a penoscrotal incision or transscrotal approach is reportedly more rapid and easier than the traditional incision. These 2 approaches were evaluated to determine which one controlled male stress urinary incontinence better. MATERIALS AND METHODS We performed a re...

Management of end-stage erectile dysfunction and stress urinary incontinence after radical prostatectomy by simultaneous dual implantation using a single trans-scrotal incision: surgical technique and outcomes

Stress urinary incontinence (SUI) and end-stage erectile dysfunction (ED) after radical prostatectomy (RP) can decrease a patient's quality of life (QoL). We describe a surgical technique involving scrotal incision for simultaneous dual implantation of an artificial urinary sphincter (AUS) and an inflatable penile prosthesis (IPP). Patients with moderate to severe SUI (>3 pads per day) and end-...

Artificial urinary sphincter revision for urethral atrophy: comparing single cuff downsizing and tandem cuff placement

OBJECTIVE To compare outcomes for single urethral cuff downsizing versus tandem cuff placement during artificial urinary sphincter (AUS) revision for urethral atrophy. MATERIALS AND METHODS We identified 1778 AUS surgeries performed at our institution from 1990-2014. Of these, 406 were first AUS revisions, including 69 revisions for urethral atrophy. Multiple clinical and surgical variables w...

A multicenter study on the perineal versus penoscrotal approach for implantation of an artificial urinary sphincter: cuff size and control of male stress urinary incontinence.

PURPOSE In a single center retrospective study we previously reported superior dry rates and fewer artificial urinary sphincter revisions when the sphincter cuff was placed via the traditional perineal approach compared with a penoscrotal approach. A multicenter study was performed to compare the approaches further and explain the disparity in outcomes. MATERIALS AND METHODS We performed a re...
